    Understanding Clozapine

Clozapine is widely recognized as an effective antipsychotic solution for patients with psychotic spectrum conditions. Clozapine therapy can help reduce symptoms, lower the risk of suicide, and improve long-term outcomes in treatment-resistant schizophrenia and other psychotic disorders.

    GET STARTED

    Overview

    What is Clozapine?

Clozapine is an atypical antipsychotic medication primarily used to treat schizophrenia, a mental illness that affects a person's ability to think, feel, and behave clearly. It's also effective in treating symptoms of psychosis associated with other serious mental illnesses (SMI) and reducing the risk of recurrent suicidal behavior in people with schizophrenia or similar disorders.

Despite its proven effectiveness, Clozapine remains severely underutilized worldwide. Many patients wait years before being considered for this life-changing option, if they are offered it at all. At Doro Mind, our Medical Director, Dr. Robert Laitman, believes every eligible patient should have safe, timely, and patient-centered access to Clozapine.

    Dr. Laitman's Approach

Our EASE Protocol for Clozapine Use

Dr. Laitman developed and uses the EASE protocol to optimize Clozapine treatment and ensure better patient outcomes. This clear and structured approach moves beyond simply prescribing Clozapine. The EASE protocol focuses on a comprehensive strategy that includes ongoing monitoring, side effect management, and active engagement with patients and their support systems.

    E

    Early Introduction

Dr. Laitman advocates for using Clozapine earlier in the course of illness, not just as a last resort after other treatments have failed. Evidence suggests that early intervention with Clozapine can lead to better long-term outcomes. This principle challenges the traditional idea that Clozapine is only for treatment-resistant cases. Instead, it encourages considering Clozapine as a potential first-line or early second-line treatment option.

    A

    Assertive Monitoring & Interventions

Clozapine can cause side effects, which is why close and proactive monitoring is essential. We encourage regular check-ups, blood tests, and assessments to help detect issues early and allow for timely intervention. In many cases, side effects can be effectively managed with supportive medications or adjustments to the treatment plan. This careful, personalized approach not only ensures patient safety but also supports long-term adherence by improving comfort and confidence in the treatment.

    S

    Slow Titration & Optimum Dosing

Titration involves gradually increasing the Clozapine dose to help the body adjust and reduce side effects. Our team emphasizes a slow, personalized titration to identify the lowest effective dose for each person with SMI. We also use therapeutic drug monitoring (TDM) to measure Clozapine levels in the blood to ensure they remain within the safe and effective range. This helps optimize dosing, monitor adherence, and prevent toxicity.

    E

    Engagement & Support

This aspect focuses on building a strong relationship with the patient and involving the family or caregivers in the treatment process. At Doro Mind, we work closely with families and caregivers caring for loved ones with serious mental illness, offering education, support, and tools to advocate for timely, appropriate treatment. By empowering caregivers, we help break down stigma and promote earlier intervention.

    Dosing

Proper Use & Dosing of Clozapine

Clozapine is available as a standard tablet, an orally disintegrating tablet (which dissolves in the mouth), and a liquid suspension. Dosage can vary from person to person based on medical history, response to treatment, and other factors.

Always give your loved one Clozapine exactly as prescribed by your doctor. If you have doubts about the prescription or dosages, speak with your doctor. Never adjust your dose unless your doctor tells you to.

If you miss one dose, give it to your loved one as soon as you remember, unless it's almost time for their next dose. In that case, skip the missed dose and continue the regular schedule.

If you miss giving two or more consecutive doses, contact your doctor before restarting. Your loved one may need to begin again at a lower dose to prevent side effects.

    Safety

    Side Effects of Clozapine

While Clozapine is an effective treatment for serious mental illness, it's important to stay aware of how your body may respond. Some side effects may occur, and your care team needs to know if they become intense or don't go away.

Common symptoms to look out for include:

    • Persistent drowsiness
    • Dizziness or feeling unsteady (vertigo)
    • Headaches
    • Shaking or tremors
    • Rapid heartbeat
    • Low blood pressure or fainting episodes
    • Excessive saliva or dry mouth
    • Sweating more than usual
    • Blurred or changing vision
    • Ongoing constipation, nausea, or upset stomach
    • Unexplained fever

Regular communication with your provider can help manage these side effects and keep your treatment on track.

    How We Help

Our Approach to Improving Clozapine Use

Despite its unmatched effectiveness, Clozapine is significantly underused. Many doctors are hesitant to prescribe it due to concerns often referred to as "clozaphobia" — an irrational fear rooted in outdated information, misconceptions about its risks, and apprehension about monitoring side effects. At Doro Mind, we're committed to changing the narrative around Clozapine.

    Importance of Specialized Expertise

Doro Mind, under Dr. Laitman's leadership, specializes in optimal Clozapine management. Many healthcare providers are cautious about prescribing Clozapine due to a lack of hands-on experience or formal training in managing the medication. Doro Mind collaborates with other providers to manage Clozapine use safely and effectively. Doro Mind can provide direct medication management through our Doro Care services and can also help to give guidance to outside providers through the Doro Compass program.

Streamlined Monitoring for Your Peace of Mind

Clozapine requires regular blood testing to monitor for side effects. We simplify this process by integrating it into your overall care plan.

We Handle Administrative & Systemic Aspects

The process of prescribing Clozapine is far more complicated than that of other antipsychotics. Providers must enroll patients in formal monitoring programs like the REMS (Risk Evaluation and Mitigation Strategy). Our team is experienced in managing the paperwork, navigating REMS requirements, and providing hands-on guidance throughout the process.

    We Partner With You

Consistent treatment and monitoring are key with Clozapine therapy. While some providers worry that patients may not reliably attend lab appointments or follow complex treatment regimens, we work closely with you and your loved one. We offer education, support, and strategies to make adhering to the Clozapine regimen and monitoring schedule manageable and successful.

    Stigma & Misinformation

Families are reluctant to try Clozapine due to its reputation as a 'last resort' drug. Misconceptions about its safety, or fear of needles and frequent blood tests, can contribute to resistance. We address these concerns by educating and empowering families with clear, evidence-based information. Dr. Laitman and our team take the time to explain not only the risks but also the unmatched benefits Clozapine offers in reducing psychosis, preventing suicide, managing aggression and substance use, and improving long-term outcomes.

    Lack of Institutional Support

Many healthcare settings lack the infrastructure to safely and efficiently initiate and maintain Clozapine therapy. At Doro Mind, we close this gap by providing a comprehensive, end-to-end care model that includes an expert provider, trained staff, streamlined lab coordination, and continuous monitoring and follow-up.
